== Early life == Henriksen was born on May 5, 1940 in Manhattan, New York. His father, James Henriksen, was a Norwegian merchant sailor and boxer, who spent most of his life at sea, while his mother, Margueritte Werner, struggled to find work as a dance instructor, waitress and model. His parents divorced when he was two years old, and his mother struggled to raise him and his half-brother Walter, leading to his spending part of his childhood in foster care. During an interview, Henriksen recounted how, at the age of seven, his mother handed him his birth certificate and said, "You'll always know who you are", then pushed him out of his home. Henriksen did not actually leave home until he was 12, saying he'd "had enough" of his home life, and that he had been physically assaulted by multiple maternal family members: "I got bludgeoned a lot. The clinical course of IPF can be unpredictable. IPF progression is associated with an estimated median survival time of 2 to 5 years following diagnosis. The 5-year survival for IPF ranges between 20 and 40%, a mortality rate higher than that of a number of malignancies, including colon cancer, multiple myeloma and bladder cancer. A multidimensional index and staging system has been proposed to predict mortality in IPF. The name of the index is GAP and is based on gender [G], age [A], and two lung physiology variables [P] (FVC and DLCO) that are commonly measured in clinical practice to predict mortality in IPF. The highest stage of GAP (stage III) has been found to be associated with a 39% risk of mortality at 1 year. This model has also been evaluated in IPF and other ILDs and shown good performance in predicting mortality in all main ILD subtypes. A modified ILD-GAP Index has been developed for application across ILD subtypes to provide disease-specific survival estimates. In IPF patients, the overall mortality at 5 years rate is high but the annual rate of all-cause mortality in patients with mild to moderate lung impairment is relatively low. This is the reason why change in lung function (FVC) is usually measured in 1-year clinical trials of IPF treatments rather than survival. === Hyperglycemia hyperosmolar state (HHS) === hyperosmolar non-ketotic state (HONK) or Hyperglycemia hyperosmolar state (HHS) is an acute complication sharing many symptoms with DKA, but an entirely different origin and different treatment. Oppositely, the prevalence of HHS is common in individuals with T2D. Furthermore, it showcases approximately ten times greater mortality rate than the observed in DKA. Both DKA and HHS occur when insulin becomes less effective, either due to a shortage of insulin secretion ( as in DKA), or lack of proper insulin action (as in HHS). For a person with very high blood glucose levels (usually considered to be above 30 mmol/L (600 mg/dL), that will result in osmotic diuresis, water is osmotically drawn out of cells into the blood and the kidneys eventually begin to dump glucose into the urine. This results in a loss of water (which contains electrolytes and glucose) that will increase blood osmolarity. If the fluid is not replaced, by mouth or intravenously, will ultimately result in dehydration (which in HHS typically becomes worse than DKA). Also causes electrolyte imbalances which are always dangerous. A decline in consciousness levels is primarily attributed to an increase in plasma osmolality. Lethargy may ultimately progress to a coma which is more common in T2D than T1D. HHS, unlike DKA, does not result in significant ketosis and acidosis, or there may be only a very minimal. This is because the presence of a small quantity of insulin suppresses the release of counterregulatory hormones and limits the production of ketones.
